Hospital Discharge Survey
As part of our work focus this year we are looking into hospital discharge and people's experiences once they have left hospital. Whether it was a planned admission (such as going in for surgery, delivering a baby etc) or the admission was due to being admitted through A&E, we would like to find out if your discharge was discussed and agreed with you and how much involvement you had with it.
We also want to know if there was any follow up appointments or support given to you when you did leave hospital and if this was agreed and arranged before you left the hospital and if your friends/family were able to be involved in this care.
The second part of the survey (which is only applicable if you used it) is about the discharge lounge and your feedback on how comfortable you felt here.
The survey will be closed at the end of August 2023, so please have your say if you have experience of hospital discharge.
